随着部队实战化训练向纵深推进,科学组训、精准防护的重要性愈发凸显。即日起,本版推出系列报道,聚焦关节健康的维护与伤病防治,邀请相关专家与基层骨干,为战友们提供实用的“关节养护指南”,引导广大官兵树立“主动防护、科学康复”的理念,以更加强健的体魄投身练兵备战。 ——编 者 科学防范髋部训练伤 ■何红艳 李春宝 宋紫逸 绘 髋部俗称胯,是连接腰腹与大腿的重要关节,承担着人体负重、行走、跑跳、屈伸等关键功能。官兵开展队列、障碍、长跑、负重等课目训练时,髋关节反复承受拉伸、扭转、冲击负荷,容易出现损伤。 官兵常发的髋部损伤,大多是日积月累的劳损所致,初期多表现为轻微酸痛、活动僵硬;若忽视防护,易发展为滑膜炎、肌肉劳损、关节退变等问题,严重时会出现活动受限、行走疼痛,影响训练质效和日常战备。 不科学的训练方式,是引发髋部损伤的首要诱因。热身不到位、动作不规范、长期错误发力等,会导致髋关节周围肌肉、韧带受力失衡,加剧关节磨损、诱发劳损损伤。因此,训练要讲科学。 训练前有效热身。针对性开展髋部专项激活训练,通过髋关节屈伸、外展、内收、环转等动作,充分放松髋屈肌、臀肌、髂胫束等关键肌群,唤醒关节活性、提升肌肉柔韧性,避免肌肉僵硬发力造成拉伤。 训练中循序渐进。严格遵循军事体育训练规律,杜绝盲目加量、带伤参训。开展长跑、负重奔袭、400米障碍等高强度训练时,规范发力姿势,避免步幅过大、腰部代偿、单侧发力,减少髋关节瞬时冲击力。根据自身体能状态合理调整训练强度,新兵及体能薄弱的官兵要逐步提升训练难度,不要一次性超负荷训练。 训练后及时放松。高强度训练后,髋关节及周围肌群处于疲劳、紧张状态,需通过静态拉伸、轻柔按摩、放松拍打等方式缓解肌肉痉挛、排出代谢乳酸,避免肌肉僵硬粘连,形成慢性劳损。 除了训练不科学外,肌力薄弱、关节稳定性不足,也会导致训练中关节受力不均、负荷过载,进而引发髋部损伤。常态化开展针对性肌力训练,是预防髋部损伤的有效手段。 髋关节的稳定性,主要依靠周围臀大肌、臀中肌、髋屈肌等肌群支撑。官兵可利用课余时间开展低成本、易操作的功能性训练。如臀桥训练可强化臀大肌力量,提升髋部负重能力;蚌式开合、侧卧抬腿可锻炼臀中肌,改善骨盆稳定性,规避关节代偿损伤;侧向螃蟹走能增强髋部外展肌力,优化下肢发力模式。训练时要遵循少量多次的原则,每组15~20次、每次3~4组,循序渐进提升肌力。 此外,还要兼顾核心力量训练,如通过平板支撑、站姿核心收紧训练等,强化腰腹核心控制力。腰腹核心力量稳固,可有效分担髋关节承重压力,纠正训练中腰部代偿、胯部代偿等不良发力习惯,降低髋部损伤风险。需要注意的是,肌力训练以适度提升身体机能为目的,避免高强度负重深蹲等过度拉伸动作,防止造成反向损伤。 不良日常习惯、长期劳损积累,也是诱发髋部疼痛的重要原因。建议官兵树立常态化防护意识,注重日常关节养护。 日常生活中注意端正体态,杜绝长期跷二郎腿、单腿负重站立、歪坐斜靠等不良姿势。此类姿态会导致髋关节长期受力不对称,造成肌群紧张、关节错位,日积月累容易引发慢性疼痛。伏案久坐时,每40分钟起身活动,做简单的髋部拉伸运动,缓解关节压迫与肌肉僵硬。 髋关节周围血液循环较慢,受凉后血管收缩、肌肉痉挛,会大幅提升劳损、疼痛、滑膜炎的发病概率。建议战友们根据天气情况和训练实际及时增减衣物,避免腰胯直吹冷风。同时,保持科学作息与均衡饮食,适量补充钙质和维生素,强化骨骼与软组织机能,提升关节抗劳损、抗冲击能力。 髋部损伤具有渐进性、累积性的特点,早期轻微劳损通过休养和防护即可快速恢复;若拖延处置,易发展为顽固性伤病。建议大家精准识别身体预警信号,树立“早发现、早干预、早治疗”的防护理念。 若出现训练后髋部轻微酸胀、久坐起身关节僵硬、小幅活动隐痛等轻度症状,需第一时间降低训练强度,暂停负重、跑跳、深蹲等高强度课目,促进劳损恢复,严禁带伤坚持高强度训练。 如果出现髋部反复疼痛、行走和下蹲受限、臀部肌肉酸胀无力、关节活动弹响疼痛等症状,说明损伤已持续加重,需及时停止训练、就医检查,明确损伤类型后对症治疗。若伴随关节肿胀、皮肤温度升高、持续性剧痛等情况,需要立即就诊,排查感染、关节退变等问题,避免病情恶化。 髋关节是下肢发力的“重要枢纽”,直接关系官兵体能水平和训练安全。建议广大官兵树立科学施训理念,走出“重训练、轻防护”“重突破、轻休养”的误区,把髋部防护融入日常,通过科学热身、强化肌力、规范养护、及时干预等措施,规避髋部训练伤发生。
